UB ADLs:
-Item: Watch
-Activity: Patient can wear a watch to keep track of time and when to take a rest break. They can also utilize it to time the length of rest breaks.

will complete EOB UB dressing using AE PRN with Mod A by 2 weeks.
-Item: Gallon of Milk
-Activity: Pt. will work on trunk stability to increase safety for sitting EOB. Pt. will utilize a gallon of milk for resistance while complete trunk twisting exercises. - Pt.
will complete UB bathing with Min A requiring less than two verbal cues by 1
week.
- Item: Sponge and bucket.
- Activity: Pt can practice UB bathing by using a bucket to hold soap and water while using a sponge to wash the body with. - Pt.
will don shirt with Min A using hemi technique sitting EOB by 2 weeks.
-Item: Computer
-Activity: Client can utilize a computer to watch video demonstrations of how to complete upper body dressing utilizing hemi-technique. - Pt.
will brush teeth standing at sink with Min A for safety by 3 weeks.
-Item: Toothbrush and toothpaste.
-Activity: Pt. could utilize a toothbrush and toothpaste while practicing completing this tasks while standing.
- Pt.
will complete LB dressing EOB with Mod A by 2 weeks.
-Item: Clothing and table.
-Activity: Items of clothing can be placed on table and patient is required to reach to obtain the clothing in order to work on increasing core strength for stability and safety while seated at the EOB. Table will be placed a distance away that requires the patient to reach outside of their base of support that requires engagement of abdominal muscles, but is still safe. - Pt.
will complete LB bathing with Mod I, implementing energy conservation
techniques by 4 weeks.

by 2 weeks.
–Item: Kitchen tongs
-Activity: Pt can use kitchen tongs as a reacher in order to grasp LB clothing and lower them to the floor without bending over and breaking their hip precautions. - Pt.
will complete LB peri-neal care during toileting activity with Min A by 2
weeks.
-Item: Side table
-Activity: Patient can have a stable surface, such as a side table placed next to the toilet seat so that they have a steady surface to hold onto for safety during standing for peri-neal care.
UB Strength:
- Pt.
will complete functional mobility in WC to dining hall with Mod I to increase
UE strength by 2 weeks.
-Item: Post-it notes
-Activity: patient will go around the house in WC short distances retrieving post it notes placed at various locations. This will work on increasing ability to maneuver in the wheelchair as well as work on UE strength by having them reach up to retrieve the post it notes. - Pt.
will increase UE strength 1 grade for increased safety and independence in ADL
tasks by 4 weeks.
-Activity: Pt can use water bottles as dumbbells in order to complete UE exercises to increase strength. They can have various marks on the water bottle indicating different water levels. The more water put into the bottles, increases the amount of resistance so they can progress the resistance and increase their strength.

will demonstrate HEP for increased UB strength with S and less than 2 verbal
cues by 1 week.
-Item: Poster board and markers
-Activity: Patient can use a poster board and markers to list and/or illustrate the exercises that are a part of the home exercise program. They can use this to demonstrate understanding of exercises as well as have it displayed as a visual cue to engage in the exercises on a regular basis and help them remember all of them. - Pt.
will increase grip strength by 3lbs to increase independence in ADL’s by 3
weeks.
-Item: Sponge
-Activity: Pt. can use a sponge to fill with water and then squeeze out in order to increase hand and grip strength.
- Pt.
will complete toilet transfer with Min A for safety by 4 weeks.
-Item: canned food
-Activity: Client can use canned food as weights to complete upper extremity strengthening exercises in order to increase independence and safety during transfer to toilet. - Pt.
will complete stand pivot transfer EOB>WC with Min A by 2 weeks.
-Item: Belt
-Activity: A belt can be put around the patient to use as a gait belt for increased safety while practicing stand pivot transfers. - Pt.
will transfer to WC with Mod I using sliding board technique by 2 weeks.
-Activity: Client can utilize a piece of plywood as a sliding board to assist in completion of a sliding board transfer.
- Pt.
will transfer from WC> shower bench for bathing activity with CGA for safety
by 3 weeks.
-Item: Bath mat
-Activity: Client can position a non-slip shower mat outside the tub so that they can have a non-slip surface when transferring in and out of the shower to increase safety.
Endurance/Energy Conservation:
- Pt.
will increase activity tolerance to 5 min to increase overall safety and
independence for self-care by 2 weeks.
-Item: Timer/Stopwatch
-Activity: A stopwatch can be used to keep track of amount of time they have been engaging in an activity as a way to measure current standing activity tolerance. - Pt
will implement energy conservation techniques for completing multi-step cooking
task with Min A, requiring <2 verbal cues by 4 weeks.
-Item: Chair
-Activity: Patient can place a chair at the counter or table so that they can sit as much as possible during meal prep in order to conserve energy. - Pt.
will complete functional activity while standing with CGA for safety, requiring
less than 3 therapeutic rest breaks by 2 weeks.
-Item: Counter top
-Activity: Client can engage in any functional task while standing at the counter. It is beneficial to be doing the task at the counter so that they will have a stable surface for added support and safety if needed. - Pt.
will verbalize 2 energy conservation techniques with Min A to use during ADL
task by 1 week.
-Item: Piece of paper and pencil
-Activity: Patient can write down energy conservation techniques so that they can remember and look over them until they know them by memory.
